Why Use Physio at Home? Supporting Case Managers With Rehabilitation

For clients recovering from significant injury, trauma, surgery or illness, effective rehabilitation is about much more than a series of physiotherapy appointments. It is about helping someone make meaningful progress in their everyday life — regaining independence, rebuilding confidence and ultimately getting back to the things that matter to them.


At Physio at Home, we provide experienced, personalised physiotherapy and rehabilitation in clients’ own homes and local communities across Mid Cornwall. We also work closely with case managers and other professionals to provide practical, goal-focused rehabilitation that complements the wider rehabilitation plan.


Between us, Ben Proctor and Hannah Trebilcock bring more than 40 years of physiotherapy experience, covering community rehabilitation, orthopaedics, neurological rehabilitation, amputee rehabilitation, significant trauma, post-hospital rehabilitation and musculoskeletal conditions.


We are both HCPC registered and fully professionally insured, giving case managers confidence that their clients are receiving care from experienced and appropriately qualified professionals.


Rehabilitation that goes beyond the home


One of the advantages of a community-based service is that rehabilitation does not have to stop at the front door.


We can assess what a client needs to achieve and then help identify practical opportunities within their local community to support those goals. Where appropriate, this can include finding a suitable local gym, helping establish links with personal trainers or other professionals, and supporting the client as they gradually progress towards greater independence.


We can therefore become an additional pair of eyes and ears for the case manager, helping turn rehabilitation goals into practical steps.


Supporting an amputee client to regain independence


We have worked with an amputee client where the rehabilitation goal extended beyond simply improving physical function.


We supported the client with becoming more independent within their own home before progressing rehabilitation into a local gym environment. We sourced a suitable gym and helped establish a link with a personal trainer so that the client could continue their rehabilitation and exercise programme with appropriate support.


This provided a pathway from home rehabilitation → community access → gym-based rehabilitation, helping the client continue progressing beyond the initial physiotherapy intervention.


Supporting a young person following a significant road traffic accident


We have also supported a young client following a significant road traffic accident.

Rehabilitation included home-based physiotherapy, alongside work on scar management and functional recovery. As their rehabilitation progressed, we looked beyond the immediate home environment and helped identify a local gym that could accommodate their needs and provide an appropriate setting for continued rehabilitation.


This type of approach can be particularly valuable following significant trauma, where recovery can involve multiple stages and the client may need support to gradually rebuild confidence as well as physical capacity.


Helping a housebound client regain independence and access the community


Another client was largely housebound and experiencing significant limitations with mobility.

We established a programme of home-based rehabilitation focused on improving her mobility and independence within her own home. As her confidence and physical ability improved, we also supported her in looking at practical ways to get back outside and access the community, including assisting with accessing a suitable car.


For this client, rehabilitation was not simply about improving a particular physical impairment. It was about helping her regain some of the freedom and independence that had been lost.


Progressing a client following significant road traffic accident


We have also worked with a client following significant trauma who initially required walking aids.


Through progressive rehabilitation, we were able to support him in progressing to the point where he was no longer using his walking aids. We then helped him move towards the next stage of his recovery by introducing him to a local gym and personal trainer, while continuing to oversee his rehabilitation and progression.


This provides an example of how we can support a client throughout different stages of recovery rather than simply providing a short period of physiotherapy.


Working collaboratively with case managers


We understand that case managers need clear information about a client's current presentation, rehabilitation needs, progress and future goals.


We can work collaboratively with the wider rehabilitation team and communicate with case managers, occupational therapists, GPs, consultants, personal trainers and other professionals where appropriate.


Our role can include:


  • Initial assessment within the client's home and wider environment

  • Identification of rehabilitation goals and functional priorities

  • Development of an individualised rehabilitation programme

  • Home-based rehabilitation and exercise progression

  • Functional mobility and independence training

  • Rehabilitation following significant trauma and road traffic accidents

  • Orthopaedic and post-operative rehabilitation

  • Amputee rehabilitation

  • Neurological and post-hospital rehabilitation

  • Scar management where appropriate

  • Supporting progression into community and gym-based rehabilitation

  • Identifying suitable local facilities and professionals to support ongoing rehabilitation

  • Liaising with personal trainers and other members of the rehabilitation team

  • Ongoing review and progression of rehabilitation goals

  • Advice and reassurance for clients and their families


Clear reporting for the wider rehabilitation team


We appreciate that good documentation and communication are an important part of case management.

Following an initial assessment, we can provide an initial assessment report outlining our findings, the client's current functional abilities, rehabilitation needs and proposed goals.

As rehabilitation progresses, we can provide updated progress reports documenting changes,

achievements, ongoing difficulties and recommendations for the next stage of rehabilitation.

At the appropriate point, we can also provide a final report summarising the rehabilitation undertaken, progress achieved and any recommendations for ongoing management.


This allows the case manager to have a clear record of the client's rehabilitation journey and helps ensure that everyone involved in the case understands the current position.


Rehabilitation with the individual at the centre


We believe that successful rehabilitation should be meaningful to the individual.

For one person, the goal might be to walk independently around their home. For another, it could be returning to work, accessing the local community, getting back into a gym, driving again or returning to a favourite activity.


We therefore try to look beyond the immediate physical problem and understand what the client actually wants to achieve.


Our approach combines clinical experience with practical problem solving, encouragement and reassurance. We can help clients identify achievable steps, celebrate progress and build confidence as they move through their rehabilitation.


A local service with the ability to travel further


Physio at Home is based in Mid Cornwall and regularly provides rehabilitation across the Falmouth, Penryn and Truro areas.


However, we appreciate that case-managed clients may be located further afield. We are able to travel to other areas where appropriate, subject to additional travelling costs, allowing us to support clients who may not have easy access to suitable community physiotherapy services locally.
